GENERAL DOCUMENTATION
Application form duly filled out and signed by the applicant (in the case of minors or incapacitated by the legal guardian);
Two recent, identical passport-size photographs in good condition to identify the applicant;
Passport or other travel document valid for 3 months beyond the duration of the intended stay; Photocopy of the biographical page;
Proof of regular status if you are a national of a country other than the one where you are applying for a visa that is valid for more than 3 months beyond the date of return;
Valid travel insurance to cover necessary expenses for medical reasons, including urgent medical assistance and possible repatriation (*);
Certificate of criminal record from the country of origin or from the country where the applicant has resided for more than one year (minors under 16 years of age are exempt from presenting documents relating to criminal record), with The Hague Apostille (if applicable) or legalized;
Request for consultation of the Portuguese criminal record by the Foreigners and Borders Service (SEF) (not applicable to minors under 16);
Proof of the existence of means of subsistence as defined by an ordinance from the competent members of the Government.
ADDITIONAL DOCUMENTATION FOR MINORS
When minors are not traveling with both parents or are traveling with a third person, a travel authorization must be presented from the parent with whom they are not traveling or from both, with a notarized signature, duly legalized, or a court order (when applicable).
- Photocopy of the parents' Identity Card.
(*) Please note that this requirement may be waived in the case of a Bilateral/International Agreement between Portugal and the country of origin regarding medical assistance that entitles certain beneficiaries to recognized insurance, as for example in the case of Brazil (PB4) and the United Kingdom (S1).
